Introduction

There a many ways to create questionnaires you can input BannQuiz and HTML code into the BannQuiz Questionnaire Generator on the BannQuiz web site. You can also use you favorite word processor or text editor to create the web page, then copy and paste this code into the Questionnaire Generator. This has the advantage of allowing the code to be save on your computer for when you might need to make changes.

To make your questionnaires look more desirable either build the web page with an HTML editor, like Netscape Communicator's Composer, add the BannQuiz command tags, then generate the questionnaire using the BannQuiz generator page. You can also use BannQuiz Generator to generate the questionnaire first, then save your web page and the import it into you favorite HTML editor.

If you do not have a text editor you can download Super NoteTab from http://www.notetab.com. Super NoteTab comes with clipbooks that allow you to insert HTML tags into a web page.

A clipbook the BannQuiz commands has been created and can be obtained at BannQuiz.clb.

To use this clipbook with Super NoteTab:

1. Save BannQuiz.clb in the Super NoteTab directory

2. Open the Super NoteTab program

3. Click on Tool in the menu bar

4. Click on open clickbook and select BannQuiz

Once you have finished creating your text you then can cut and paste it into the BannQuiz Questionnaire Generator as well as save for further updates. The BannQuiz Questionnaire generator can be located at:

A BannQuiz template as also been created for MSWord97 calledBannQuiz.dot.

The self-extracting version for the PC can be obtained at BannQuiz.dot.exe. Once you have downloaded this file you have to uncompress it to get the BannQuiz.dot template by double clicking on the BannQuiz.dot.exe. To use the template you must first move the BannQuiz.dot to the Microsoft Office folder. Run MSWord and Click on File then open. The open window will pop up then change the file type to "document template" then click on BannQuiz.dot

The MacIntosh macintosh version can be obtained from BannQuiz.sit. If you have Stuffit expander the file should uncomress automatically. The the procedure is the same a for the PC.

When you have finished creating the page use go to File then Save As HTML or Save as Text to create a document ready for the BannQuiz Generator.
